Präventive Arbeit mit Müttern und ihren Babys im tagesklinischen Setting

Zentralen Einfluß auf das kindliche Entwicklungsoutcome hat die gelungene Kommunikation zwischen Mutter (und Vater) und Kind in entwicklungsbezogenen Interaktionsthemen. Diese ist bei den Müttern, bei denen aufgrund ihrer Psychopathologie die Indikation zur tagesklinischen Behandlung gestellt wird, in erheblichem Maße eingeschränkt. Daneben besteht aufgrund der Ich-Schwäche nur eine rudimentäre Fähigkeit zur Flexibilität in Adaptation an die Bedürfnisse des Kindes und wegen der eigenen Bedürftigkeit eine insuffiziente Feinfühligkeit und Responsivität in bezug auf die kindlichen Signale. Bei fast allen Patientinnen bestehen aggresive Impulse dem Kind gegenüber, die, soweit sie reflektierbar sind, die Indikation zur tagesklinischen Behandlung wesentlich mitbestimmen. Dem präventiven Aspekt der Vermeidung einer frühen Bindungsstörung und/oder eines pathologischen Entwicklungsoutcomes wird nicht nur durch die angestrebte Vermeidung einer Trennung von Mutter und Kind bei Mißhandlungsgefahr, sondern auch durch die spezifischen Therapiemodule unseres Settings Rechnung getragen. Auf seiten des Kindes besteht die Indikation zur teilstationären Aufnahme in einer schweren Regulationsstörung.

Certain aspects of the communication between mother (and father) and baby are of major influence on the developmental outcome of the child. Mentally ill mothers in psychiatric day care are considerably limited in their communicative abilities, especially in themes of developmental relevance. As a consequence of their inherent ego weakness the mothers' flexible adaption to the babies' needs is impaired. Because of their own emotional neediness mothers are not sufficiently sensible and responsive to the babies' signals. Almost all of the mothers feel aggressive impulses towards their babies. Indication for day care depends to an essential part on the mothers' reflective function concerning aggression. Although there is some danger of child abuse, the joint treatment is considered to prevent early attachment disorders and/or a pathologic developmental outcome in the child. In addition, mother-baby-therapy is indicated in cases of severe regulation disorders. The specific moduls of our therapy are il- lustrated in two case examples.
