Abstract / Description

Die Möglichkeit und die Anwendung der Pränataldiagnostik (PND) hat das psychische Erleben der Gravidität für die schwangere Frau, den werdenden Vater und die Gesellschaft grundlegend verändert. Beim Umgehen mit der PND muß eine Ausgewogenheit zwischen den positiven Auswirkungen für eine Vielzahl von Menschen und den Risiken und Belastungen für einen ebenfalls nicht unerheblichen Teil von Betroffenen angestrebt werden. Dabei geht es um Entscheidungen für oder gegen Untersuchungen bzw. Interventionen und deren Verarbeitung, geeignete begleitende Betreuungsstrategien sowie die Möglichkeiten professions- und institutionsübergreifender Kooperation. Durch die wachsenden Möglichkeiten nichtinvasiver Methoden entwickelt sich aus der primär individuellen zunehmend eine populationsbasierte PND. Damit dürfte die Forderung nach einer informierten Zustimmung alle Schwangeren betreffen. Der informed consent orientiert sich an der Situation, d.h. je invasiver/risikoreicher die Untersuchung oder je folgenschwerer das mögliche Resultat, desto ausführlicher muß die Aufklärung geleistet werden. Eine gute fachliche Qualifikation der Betreuenden, die sowohl fundiertes somatisches und psychosomatisches Wissen als auch die Fähigkeit zur kommunikativen Gesprächsführung und Beziehungsgestaltung und die Fähigkeit zu Teamarbeit und Kooperation einschließt, ist dafür erforderlich.
The possibility and the application of prenatal diagnostics (PND) has fundamentally changed the psychological experience of pregnancy for the pregnant woman, the expectant father, and society. When dealing with PND a balance between the positive effects for a large number of people and the risks and strains for an also considerable number of affected people must be strived for. This affects the decisions for and against examinations or interventions and coping with these, suitable strategies of care, and also possibilies of cooperation of different professions and institutions. Due to the growing number of possibilities of non-invasive methods a population-based PND is developing from the primarily individual PND. This means that the requirement for informed consent should affect all pregnant women. Informed consent is oriented on the situation, i.e. the more an examination is invasive/risky or the more serious the consequences are, the more extensive the information has to be. This requires a qualified caring person who must have well-founded somatic and psychosmoatic knowledge, must also be able to conduct communicative counselling and create relationships, and be cooperative and be able to work in a team.
