Abstract / Description
Es wurde der Einfluss des Aufnahmemodus von Jugendlichen in die stationär psychiatrische Therapie - freiwillig versus unter Zwang - auf den Therapieerfolg untersucht. Für die Erhebung wurde ein zum Teil eigenständig erstellter, zum Teil Items aus dem Inventar zur Erfassung der Lebensqualität bei Kindern und Jugendlichen (ILK) enthaltender voll strukturierter Selbstbeurteilungsbogen verwendet und katamnestisch eingesetzt. Die Eingangsbefindlichkeiten waren für die unter Zwang aufgenommenen Patienten schlechter. Unabhängig davon erzielen beide Patientengruppen einen vergleichbar guten Therapieerfolg gemessen an der selbst eingeschätzten Lebensqualität. Nachweisen lassen sich Effekte der Patientenzentrierung und der Informiertheit der Patienten auf deren Compliance und Einstellung gegenüber der Therapie. Ein positives Vertrauensverhältnis zu den therapeutischen Mitarbeitern trug ebenfalls bei beiden Gruppen zum Outcome bei. Der Aufnahmemodus hat in Hinblick auf die erreichte Lebensqualität offenbar nur eine geringe Bedeutung, oder die Bedeutung wird im Behandlungsverlauf im Rahmen der therapeutischen Vertrauensbeziehung kompensiert. Dagegen spielen die Mitsprachemöglichkeit, die Teilhabe und das Gefühl der Patienten, ernst genommen zu werden, eine zentrale Rolle für die Verbesserung der Compliance und als Moderatorvariable auch für den Therapieerfolg. Die Optimierung der Partizipation minderjähriger Psychiatriepatienten wurde mit den Ergebnissen als konstituierend für positive Behandlungsergebnisse belegt.

We examined the influence of the admission mode of adolescents into inpatient psychiatric therapy - voluntary or involuntary - on the outcome. For statistics, a fully structured self-assessment form was applied some months after dismission, partly drawn up independently and partly containing items from the inventory for quality of life in children and adolescents (ILK). The initial psychological state was less favourable in patients who were admitted under constraint. However, both patient groups showed a comparable outcome. Positive effects of the patient information on their compliance and attitude towards the therapy were demonstrated, as well as a positive influence of the confidence in the therapeutic staff. Regarding the quality of life obtained at the end of treatment, the admission mode is apparently less important or is put into perspective by staff-patient-relations within the therapeutic relationship. On the other hand, the possibilities for participation and the patients' feeling to be taken seriously play a major role in improving compliance and, as a moderator variable, for therapeutic success as well. The results underline the importance of improving participation processes of hospitalized adolescent patients in psychiatry. This seems to be constitutive for a positive long-term treatment outcome.
