Abstract / Description

Einleitung: Leitsymptome der Fibromyalgie (FM) sind lokalisierbare Schmerzen an den sog. "tender points" sowie Ganzkörperschmerzen. Zusätzlich klagen die Patienten über eine Vielzahl anderer Symptome. Bei der Erforschung der Ätiologie dieser Erkrankung wurde deutlich, dass die aufgefundenen pathogenetischen Einflussfaktoren unterschiedlich zu gewichten sind; auch zeigen psychologische und psychobiologische Studien häufig inkonsistente Befunde. Dies unter-streicht die Notwendigkeit der Subgruppendifferenzierung. Ziel der vorliegenden Arbeit ist die Untersuchung von Subgruppen des Fibromyalgie-Syndroms unter besonderer Beachtung interaktioneller und emotionaler Aspekte. Methodik: Zur Subgruppenbildung wurden die Ausprägungen einer konsekutiven Stichprobe von 35 stationären FM-Patientinnen in verschiedenen Selbstratinginstrumenten (BDI, IIP-D, PSKB, SOMS, CSQ, BFKE) sowie in den OPD-Achsen Konflikt und Struktur und im mimisch-affektiven Verhalten (EMFACS) verwandt. Grundlage für die Mimikkodierung war ein vide-ographiertes OPD-Interview. Das mimisch-affektive Verhalten wurde kodiert, wenn die Patien-tinnen über ihre Beschwerden und Schmerzen sprachen. Als statistische Verfahren kamen Clusteranalysen zum Einsatz, zur Untersuchung des Einflusses der verwandten Variablen auf die Clusterzugehörigkeit wurden logistische Regressionen verwandt. Als Kontrollgruppe dienten die Daten von 20 gesunden Frauen. Ergebnisse: Die Clusteranalysen führten zu einer 3-Cluster-Lösung: Subgruppe 1 (Cluster 1, N: 11): Die Patientinnen schildern im Gruppenvergleich die höchsten Werte im beruflichen und privaten Stress, in den interpersonalen Problemen, im Leistungsan-spruch, im Katastrophisieren sowie in den Depressionen und Ängsten. Ihr mimisch-affektives Verhalten ist durch die höchsten Werte in negativen Affekten (v.a. Ekel und Verachtung) in und außerhalb des Blickkontaktes dominiert, Leitaffekt im Blickkontakt ist Ekel. Die Patientinnen aus Subgruppe 1 zeigen die meisten Abweichungen im Vergleich zu den gesunden Frauen. Prognostiziert werden kann diese Cluster-Zugehörigkeit durch viele Affekte im Blickkontakt (insbesondere durch starken Ekel) sowie durch negative Affekte und deutliche Ängste. Subgruppe 2 (Cluster 2, N: 6): Die Patientinnen schildern die niedrigsten Werte in der Schmerzstärke, der Gesamtbeeinträchtigung, im Katastrophisieren, im Stress, in den ärztlichen Behandlungen sowie die niedrigste Dauer der Arbeitsunfähigkeit und die geringste Häufigkeit eines Rentenwunsches. Im mimischen Verhalten zeigen sie in und außerhalb des Blickkontaktes am häufigsten Freude, dieser Affekt ist auch Leitaffekt. Insgesamt weisen ihre Merkmalsausprä-gungen und auch ihr mimisch-affektives Verhalten die größte Nähe zu den gesunden Frauen auf. Prognostiziert werden kann diese Zugehörigkeit durch viel Freude im Blickkontakt und durch eine niedrige Gesamtbeeinträchtigung. Subgruppe 3 (Cluster 3, N: 18): Die Patientinnen schildern die höchsten Werte in der Schmerz-stärke und der Gesamtbeeinträchtigung sowie die höchste Häufigkeit eines Rentenwunsches (80% der Patientinnen). Dominiert wird ihr mimisches Verhalten durch einen sehr hohen Wert an sozialem Lächeln sowie durch eine Reduktion von Affekten im Blickkontakt. Prognostiziert werden kann diese Cluster-Zugehörigkeit durch das Fehlen von Affekten im Blickkontakt sowie durch einen vorhandenen Rentenwunsch. Schlussfolgerung: Die Selbstbeschreibungen der Patientinnen aus Subgruppe 1 weisen auf er-hebliche Belastungen im privaten und krankheitsbezogenen Bereich hin. Während ihrer Schmerzklage vermitteln sie einen starken Leidensdruck, zum anderen implantieren sie affektiv Distanzwünsche und Entwertungen in die Beziehung. Es ist zu erwarten, dass diese Art der Schmerzklage zu erheblichen Schwierigkeiten in den Arzt-Patienten Beziehungen führt. Psycho-therapeutisch scheinen für die FM-Subgruppe vor allem psychodynamisch-interaktionelle Ver-fahren indiziert zu sein. Subgruppe 2 ist durch die geringste Ausprägung psychopathologischer Faktoren und Beeinträch-tigungen gekennzeichnet. Ihr mimisch-affektives Verhalten ist durch eine hohe Beziehungsstabi-lisierung und eine große Nähe zum gesunden Verhalten gekennzeichnet. Psychotherapeutisch scheinen vor allem Schmerzbewältigungsprogramme indiziert zu sein. Subgruppe 3 ist durch Selbstbeschreibungen gekennzeichnet, welche auf hohe krankheitsbezo-gene, aber nur geringe psychische Belastungen hinweisen. Auffällig wird vor allem ihr Wunsch nach Rückzug vom Erwerbsleben. Ihr mimisch-affektives Verhalten weist durch die mangelnde affektive Besetzung auf eine Ausdünnung des emotional-interaktiven Involvements hin. Psycho-therapeutisch scheinen diese FM-Patientinnen aufgrund des Rentenbegehens schwer zugänglich zu sein, sodass erst eine gezielte Förderung der Therapiemotivation, welche z.B. mittels einer ambulanten und psychoedukativ-orientierten Gruppentherapie geschehen könnte, indiziert wäre.
Introduction: Fibromyalgia syndrome (FM) is a musculoskeletal pain disorder characterized by chronic widespread pain and hypersensitivity to palpation at specific tender points. In addition there is a possible range of comorbid symptoms. A considerable variability within individuals matching diagnostic criteria is striking and psychological and psychobiological studies often show inconsistent results. Therefore the differentiation of FM-subgroups seems to be necessary, ideally using features and constructs that would help to specify the therapy of fibromyalgia. The aim of the present study was the investigation of FM-subgroups particularly taking emotional and interactive aspects into account. Method: To differentiate subgroups the values of a consecutive sample of 35 inpatient female FM-patients in some self-rating inventories (BDI, IIP-D, PSKB, SOMS, CSQ, BFKE), in the OPD-axis structure and conflict as well as in their facial-affective behaviour (EMFACS) were used. Base for the coding of facial expressions was a video-taped OPD-interview. Facial expressions were coded when patients reported on their pain and complaints. Statistical proce-dures were cluster analysis, the impact of variables on cluster membership was analysed by logistic regressions. As control group data from 20 healthy women were used. Results: Cluster analysis resulted in a 3-cluster-solution: Subgroup 1 (Cluster 1, N: 11): patients report in comparisons to the other groups the highest levels of vocational and private stress, interpersonal problems, action proneness, catastrophizing as well as the highest levels of depression and anxiety. Their facial affective behaviour is characterized by the highest expressions of negative affects (above all: disgust and contempt), their "leading affect' in mutual gaze is disgust. These patients show the most discrepancies in comparison to the control group. The membership of this cluster is predicted by many affects in eye contact, above all by strong disgust. Negative affects and high anxieties increase the prob-ability to be a member of this subgroup, too. Subgroup 2 (Cluster 2, N: 6): patients report in comparisons to the other groups the lowest values in pain intensity, impairment, in catastrophizing, in vocational and private stress as well as in medical treatments, the lowest duration of sick leave and the lowest frequency of a wish for early retirement. Their facial affective behaviour is characterized by the highest expressions of genuine joy and this is their "leading affect', too. These patients are closest to the healthy women. The membership of this cluster is predicted by high expressions of joy and by low impairment. Subgroup 3 (Cluster 3, N: 18): patients report in comparisons to the other groups the highest pain intensity and impairment as well as the highest frequency of a wish for early retirement (80% of patients). Their facial affective behaviour is dominated by a very high expression of social smile and by a reduction of affects in mutual gaze. The membership of this cluster is predicted by the lack of facial expressions in eye contact and by the wish for early retirement. Conclusions: Self-descriptions of FM-patients in subgroup 1 indicate considerable illness-related and private strains. During complaining they communicate on the one hand an intense psychological strain, on the other hand they affectively implant dissociating and devaluating elements in the interactions. This kind of complaining could probably result in considerable difficulties in the physician-patient relationships. A psychotherapeutic approach for this FM-subgroup should include psychodynamic elements. Subgroup 2 is characterized by low psychopathological factors and impairments. Their facial affective behaviour is marked by stabilizing elements and is close to healthy behavior. A psychotherapeutic approach for this FM-subgroup should include pain-coping elements. Subgroup 3 is marked by self-descriptions indicating high illness-related, but only few psychological strains. Striking is their wish for early retirement. Their facial affective behaviour, that is characterized by low affective cathexis indicates a thin emotional-interactive involvement. Because of their wish for retirement these patients could hardly be reached by psychotherapy, so a well directed encouragement of therapeutic motivation by e.g. an ambulant educational group therapy could first be indicated.
